  2. break down into job steps
  3. observe the examine risks
  4. develop control measures
  5. formulate safe system of work
  6. review and maintain

criteria include:

  • accidents / loss history
  • potential severity
  • probability of recurrence
  • legal requirements
  • newness of the job
  • number of workers at risk

safe systems of work:

  1. safe design
  2. safe installation
  3. safe premises and plant
  4. safe tools and equipment
  5. correct use of plant, tools and equipment (training and supervision)
  6. effective planned maintenance of plant and equipment
  7. proper working environment ensuring adequate lighting, heating and ventilation
  8. trained and competent employees

  11. additional protection for vulnerable employees
  12. formalized issue and proper utilization of all necessary PPE
  13. continued emphasis on adherence to the agreed safe method of work
  14. regular reviews of all written systems of work.
  15. regular feedback to all the concerned-possibly by safety committees and job contact training sessions-following any changes in existing systems of work.

job step - risk factor - control action

system safety:

  • originated in aerospace/aviation
  • concern: whole system safety, not just tasks
  • it involves: hazard identification, safety in design, continuous monitoring

Method analysis:

  • hazard and operability (HAZOP)
  • technique of operation review (TOR)
  • Gross Hazard Analysis
  • Classification Of Risks
  • Risk Ranking
  • Failure of Modes and Effect Analysis
  • Fault Tree Analysis
